He wears wears a red mask with scratch marks that spell out FREAK. Even though the mask seems to be on by a strap it actually head by little needles that touch his skin, none of the needles are straight as there crooked and curved to serve one purpose to cause him pain (read back story for explanation). While the masks only has one real purpose which is only to hide his 'smile.'
His unique smile has to do with a device that he created. A metal contraption the goes right where his lips end when he smiles and stretches to the bottom of his ear. Going in a curved direction where there's small metal arms that hold a razor. With every smile tiny pressure plates that move the arms cutting the delicate skin. More little arms come and stretch the skin giving a extended smile. As soon the smile stops the arms go back and the razor lifts a little as the other arms quickly stich the skin close again.
He has a likes to wear a black hoodie with barbed wire on top of it. Its wrapped around from his wrists to his shoulders the sharp ends prick in his skin every time he moves. While having a small vest underneath the hoodie holding a few bundles of much needed barbwire in case the ones on his arms aren't enough for a fight.
Back story: Being an average Joe in the city of nackel. You know the common nobody, after a car accident with a truck from the genation company. An experimental virus breaks open, exposing the poor man who was in significant pain. However after minutes of being exposed the virus has helped stop the bleeding and close alot of the wounds. But with an unknown side effect, that pain became a weird pleasurable sensation. It gave him a high that no drug could compare to. But with various testing he's still a mere mortal bones took days to heal and his blood doesn't just replenish itself instantly. This made it hard for him safely experience his pain high without nearly killing himself. It drove him mad and he started to experiment with making various pain devices however the smile device was made it was a perfect match. It was enough pain for him. To achieve the high while not breaking any bones or risking his life.
